				<title>Alterations in Interleukin-8, High Sensitivity C-reactive Protein and Some Hematological Parameters Uncover Ongoing Pathologic Process in Bakery Workers Exposed to Flour Dust</title>

				<abstract_fa><![CDATA[]]></abstract_fa>
				<abstract><![CDATA[Background: Exposure to flour dust has been linked to the emergence of respiratory symptoms. This study assessed the levels of some inflammatory markers and hematological parameters among bakery workers exposed to flour dust in Ota, Nigeria.
Methods: A total of 100 participants (50 bakery workers with exposure to flour dust and 50 non-bakery workers without such exposure) took part in this study. The participants&#039; peak expiratory flow rate was measured using a peak flow meter. Blood samples were assessed for high sensitivity-CRP, interleukin-8, and a complete blood count. The collected data was analyzed using SPSS version 23.0.
Results: The outcomes revealed that bakery workers demonstrated significantly (p≤0.05) elevated levels of high sensitivity-CRP, interleukin-8 concentrations, red blood cells, hemoglobin concentration, packed cell volume, platelets, and lymphocytes compared to non-bakery workers (p≤0.05). Meanwhile; the mean absolute neutrophil count and neutrophil-lymphocyte ratio of the bakery workers were found to be significantly (p≤0.05) lower compared to non-bakery workers.
Conclusion: This study underscores the health risks faced by bakery workers exposed to flour dust, as evidenced by alterations in inflammatory markers and hematological parameters. These findings highlight the ongoing pathological processes induced by flour dust exposure, emphasizing the need for preventive measures and occupational health interventions in bakery settings. Enhanced awareness and protective measures are imperative to mitigate the adverse health effects associated with flour dust exposure in occupational environments. Further research is warranted to elucidate the mechanistic pathways underlying these observed alterations and to develop targeted interventions for mitigating occupational health risks in bakery workers.

				<title>Investigation of Heavy Metals, Nitrate, and Nitrite in The Commonly Available Commercial Packed Drinking Water in Mashhad, Iran</title>

				<abstract><![CDATA[Background: Due to the increasing consumption of bottled water in recent years, to ensure the safety, we aimed to evaluate the concentration of 6 toxic heavy metals, nitrate and nitrite components in the commonly sold bottled water in Mashhad, Iran.Methods: The 11 best-selling bottled water brand in Mashhad were identified. Eight bottles from each brand were randomly collected and delivered at the same day to toxicology laboratory of Imam Reza Hospital and refrigerated at 4-6 °C. Spectrophotometry and atomic absorption spectrometry and were used to measure the nitrate, nitrite and heavy metals, respectively. The results were analyzed by SPSS version 16 and compared with the WHO and Australian guidelines. Also, the discrepancy between measured components and the depicted labels’ values were compared.Results: The mean and SD of concentrations of the heavy metals in 11 brand were as below: lead 1.62±0.86 [µg/L], chromium 1.03±0.84 [µg/L], cadmium 0.17±0.07 [µg/L], mercury 3.86±1.57 [µg/L], arsenic 0.89±0.46 [µg/L], aluminum 6.56±4.54 [µg/L]. The mean and SD measured quantities of nitrate, nitrite and pH were 9.96±5.95 [mg/L], 0.01±0.03 [mg/L] and 7.92±5.95, respectively. There was a significant difference between the label values and the quantitative levels except for 3 brands, which was observed with a p value of 0.518 and 0.642 for nitrate level in N4 and N11 brands, as well as 0.681 for pH level in N7 brand. The measured values of heavy metals, nitrate and nitrite in all samples were within domestic, WHO and Australian limits, except for mercury in 9 samples which exceeded the Australian standard [less than 1 µg/L].Conclusion: The heavy metals, nitrate and nitrite concentrations in all samples were within the domestic, WHO and Australian ranges, except for mercury in 9 samples that exceeded the Australian standard.  There was a discrepancy between entries of the bottled labels and the measured quantities.]]></abstract>

With the ongoing advancements in artificial intelligence (AI) and its increasing integration into medical and pharmaceutical domains, there is a growing interest in exploring the role of AI in the context of poisonings. This study aims to investigate the potential applications of AI in the management and treatment of poisoning.Materials and methods: This research is a review by searching the keywords (&quot;Artificial intelligence&quot;) [TIAB] AND (poisoning [TIAB] OR toxicity[TIAB] OR intoxication[TIAB] OR Toxin[TIAB] OR poison[TIAB]) was searched in the internet databases PubMed, Scopus, and Google Scholar search engine in 2016-2024.Conclusion: Enhancing system management and treatment approaches is crucial in preventing accidental and intentional poisoning. This can be achieved by incorporating cutting-edge medical equipment, such as those equipped with AI. AI technology can recognize complex patterns beyond predefined rules and process large amounts of data, exceeding human capabilities. In the future, more progress in AI will likely affect various areas of healthcare, including poison prevention and treatment, to improve patient outcomes and reduce the burden of poisoning on healthcare systems. ]]></abstract>

				<abstract><![CDATA[Background: Toxicology is a critical field that is of significant importance to various industries, including pharmaceuticals, environmental protection, and consumer product safety. It&#039;s a multidisciplinary science that often involves time-consuming and expensive toxicity tests, which can delay the development of new products and pose significant risks to public health and the environment. Therefore, there is an ever-growing demand for faster and more efficient toxicity evaluations. Artificial Intelligence (AI) has emerged as a promising solution to address these pressing challenges. By enabling the development of machine learning models that can analyze vast amounts of data. This review article focuses on the potential impact of AI in toxicology and its applications in different areas, such as predictive toxicology, development of toxicity screening assays, assessment of chemical mixtures, interpretation of toxicological data, and forensic toxicology.
Methods: This review was done a comprehensive literature search across multiple scientific databases. Searches were conducted in Medline/PubMed, Google Scholar and Web of Science to identify relevant publications. The search terms used included combinations of &quot;artificial intelligence&quot;, &quot;toxicology&quot;, &quot;toxicity&quot;, and related keywords. The final set of articles selected provided a comprehensive overview of the current state of research on the applications of AI techniques in toxicology and chemical risk assessment.
Results: The review highlighted a growing body of research exploring the potential role of AI in accelerating and enhancing various aspects of toxicity assessment and chemical risk evaluation. The reviewed studies demonstrate how AI models can be trained on large datasets of chemical structures, in vitro assay results, and toxicological outcomes to predict the toxicity of novel compounds and other fields such as forensic toxicology. On the other hand, legal and ethical aspects of using AI was investigated.
Conclusion: Overall, the findings of this review highlight this fact that AI can enable faster, more cost-effective, and more accurate toxicity assessments and ultimately leading to improved chemical safety and risk management practices. potential role of AI in accelerating and enhancing various aspects of toxicity assessment and chemical risk evaluation. 				<title>Measurement of Blood Phenobarbital Concentration in Newborns Admitted to The NICU of Imam Reza Hospital and Receiving The Drug Via Intravenous Mode</title>

				<abstract><![CDATA[Introduction: Newborns may be treated with phenobarbital for many reasons. Because in each region, depending on different races and genetic factors, different pharmacokinetic conditions govern the drug. It is essential to control blood levels of certain drugs, especially phenobarbital, and maintain these levels during treatment. In this study, we measure the level of phenobarbital in newborns who receive intravenous.Methods: In this study, venous blood was collected from 50 neonates who received intravenous phenobarbital at a loading dose of 20 mg/kg weight, and at least, three days had passed since the maintenance dose of 5 mg/kg body weight in 24 hours, and sent to the laboratory. Phenobarbital blood levels were measured, and the results were analyzed descriptively.Results: In this study, the average weight of newborns in two groups was 9.93 ± 2.58 kg. The mean blood concentration of phenobarbital, three days after starting the maintenance dose in the group of infants weighing more than 2.5 kg was 3.33 ± 9.1 micrograms/liter, in the group of infants weighing less than 2 kg. and half a kilogram or LBW was 5.9 ± 9.5 micrograms/liter, and in the group weighing less than 1.5 kg VLBW was 14.4 ± 15.46 micrograms/liter. There was no significant difference between case and control group (p&gt;0.05). Three days after starting the maintenance dose in all three groups, the mean blood phenobarbital concentration was 9.86 ± 0.86 micrograms/liter.Conclusion: Blood phenobarbital levels in our newborns are below therapeutic levels at day two, so phenobarbital levels should be evaluated.]]></abstract>

				<abstract><![CDATA[Background: Cartap is a class 4 pesticide and categorized by the insecticide resistance action committee (irac) as safe and non-toxic to people. The main routes of exposure include ingestion, skin contact, and eye contact.Case presentation: A 35-year-old man, a bus driver from South Kerala, was bought to the emergency department with history of consumption of cartap hydrochloride after two hours of poisoning.Results: All the necessary measures were taken to prevent further exposure to the poison. Patient was intubated and mechanically ventilated. The patient&#039;s condition began to deteriorate gradually, culminating in multiorgan dysfunction and eventual death on the 7th day of hospitalization, with no restoration of consciousness.Conclusion:  This report enables medical professionals to identify and manage an uncommon poisoning that causes lethal toxicity. ]]></abstract>

				<abstract><![CDATA[Introduction: Atypical antipsychotics were usually used in management of many psychiatric disorders. Moreover, suicidal attempts by antipsychotic agents are not rare in clinical practice. Therefore toxicologists should be aware of the early detection and management of those patients. Clozapine is one of the most commonly used antipsychotic drugs due to its well-known efficacy.Case presentation: this is a case report of a young previously healthy male patient who committed suicide by ingestion of 1000 mg of the atypical antipsychotic drug clozapine. The case highlights the development of severe disturbed level of consciousness, extrapyramidal adverse effects, prolonged QTc with positive troponin, hypokalemia, hyperglycemia, transient hypertension, and elevated liver transaminases. Discussion: The proper treatment initiation of acute antipsychotic poisoning should be based on the patient&#039;s clinical assessment. Early central nervous system and cardiovascular support are the core treatment element. However, the case reported uncommon electrolyte and hepatic disturbances which require early detection and monitoring.Conclusion: Atypical antipsychotics are not usually a safe treatment option for psychiatric diseases. Acute clozapine poisoning is a serious condition in which early resuscitation is essential for good prognosis. Serious cardiac and metabolic adverse effects should be considered, early investigated and properly monitored. Serum drug level might not be helpful prognostic indicator for acute antipsychotic poisoning.]]></abstract>

				<abstract><![CDATA[Introduction: Postmortem artefacts are changes that occur in a body after death, but before the autopsy, which can sometimes lead to misinterpretation of autopsy findings. These artefacts can result from various factors such as environmental conditions, the body&#039;s position, medical interventions before death, or the decomposition process. In this article we explain an artefact encountered in a case of Celphos poisoning.
Case Presentation: A 54-years-old man, a daily wage worker, brought to GTB Hospital with alleged history of ingestion of 2 tablets of Celphos and got admitted, gastric lavage was done and expired during course of treatment. In postmortem examination, stomach contained blackish fluid along with whitish oval shaped solid matter. In mortuary, dead bodies were stored in proper refrigerated storage cabinets/shelves or cold chambers. Due to that, coconut oil, which was used for resuscitation got frozen in stomach and remained in solid state.
Conclusion: Coconut oil was an iatrogenic artefact in this case. This indicates that these artefacts are present in significant number of cases, which may lead to misinterpretation of findings as evidence of underlying assault or accident if autopsy surgeon is not aware of these artefacts. Accurate interpretation is needed before labelling such findings as proofs of assault, accident or an artefact.]]></abstract>
